We started the day two nurses short and with two ICU holds so that took one nurse for them, two nurses and the charge nurse to run the rest of the ER. It wasn't long until we all had 4 patients including the charge nurse and the nurse with the two ICU holds which put us way out of ratio. We told the nursing supervisor and her response was 'ok.' The other hospital went on diversion so we started getting all the ambulances plus the doctors were accepting patients from outlying facilities. Pretty soon we all had six patients including the two ICU holds, 3 step-down holds and 3 medical floor holds. there were 18 patients in the lobby with complaints of chest pain, weakness, pneumonia and there was nothing we could do about it. Fast track was doing work ups. The nursing supervisors response? "Do the best you can, we know it is bad down there."

WTF? You know it's bad down here? Get down here and help us for God's sake.

Our charge nurse blew her top. It was pretty impressive as she is the most even tempered, easy going person I have ever know. She never even raises her voice. She said to the supervisor "I am declaring an internal disaster" and hung up before there was any response. She overhead paged "Internal Disaster, ER now."

Surprisingly, when all the directors get called in there are bed assignments. Funny how that happens. The cath lab crew was called in to work in the ER, they threw a big hissy fit at that but were told they had to come in. The president of nursing told the nursing supervisor to put on scrubs and get down to help us, which she did - wonder how long it has been since she has done any real nursing?

I got out of work 9 hours after my shift was over, which means I worked 21 hours. I am whipped. Last I heard there was going to be a big pow wow this morning with all of admin to figure out a solution to the problem we've been having the last few months. Let me help you, get some nurse in here now!

Sheesh.

I just hope it doesn't result in charge nurse out of a job because she is AWESOME!!!

Hi! I read your posts via email subscription which makes me a bit slack when it comes to actually coming here and making comment. So sorry about that!

I have read what you guys do and have to put up with and I must say I'm into total awe of your dedication and patience.

So I'll give you my thanks for the ones that you care for. For if any of you left to seek greener and calmer pastures, heaven help those that come to your front door for help.

On a bad day, I pray for six patients. Heck, I pray for anything less than twelve!

Now I feel bad for my snarly comment. I'm just cranky because I just got back from working six hours of overtime that turned into ten. Blech.
It's rough everywhere.

21 hours is a lot of hours! And illegal, no?

Well Guitargirlrn, I too have worked in places like that but I know that you can't really give very good care when you have so many patients. California ratio is 1:4. The floors never have to go out of ratio but we are expected to routinely and it chaps my ass.

O I so feel your pain, thank goodness for your charge nurse stepping up and putting her foot down! Sometimes ours just smile and say, we will do what we can. Its just not safe when that happens........

It is quite sad when I have to say that your charge is BRILLIANT!...all for doing the right thing. Gutsy gal. Give her a 'way to go!' from all of us. :-)

9 hours over?! That sounds like EMS! :-)

One of our charge nurses was recently fired for putting us on by-pass and declaring an internal disaster. The director of the ER walked right in and fired her on the spot.

And they wonder why we're losing all our nurses in the ER. I couldn't possibly be that we're continually over ratio because at our joint its all about the cash coming in and not adequate patient care.

Go charge nurse! I hope she doesn't receive the same treatment as er tech dude's charge nurse.

And to think - it had to take declaring an internal disaster for them to sack up and solve the problem you guys have been dealing with for months! How ridiculous and completely unsafe of the nursing supervisor to blow it off like that! It just goes to show that until you MAKE it their problem, nothing gets done. Kudos to your charge for having the courage to step up like that!
